Medicare/Medicaid beneficiaries and other federal/state healthcare benefit recipients -
Diagnosis of congestive heart failure, cardiovascular disease, chronic kidney disease or any other condition. -
Commercial insurance co-pay less than $65 for a 30-day supply.
